Dr Conklin argues that, because humans are fallible, accidents are inevitable. So, organisations are never going to succeed in stopping all accidents.
That means attention must be paid to protecting people when the inevitable accidents occur. This requires leaders to focus on building organisations that can ‘fail safely’.
